Form Purpose
This form enables you to request a re-visit for the purposes of re-rating the food hygiene at your establishment.
As the food business operator of the establishment you have a right to request a re-visit for the purposes of re-rating if you have taken action to rectify the non-compliances identified at the time of inspection.
Form Submission
When you have completed the form please ensure you submit it. On submission you will receive an email response at the email address you provide in the form.
Payment
All requests for a re-visit using this form require a payment of 215 GBP. The form will only be processed after payment has been received. You can pay online when you submit your application. Alternatively, you can pay over the phone.
